What is Care Management?
Care Management is one element and a vital part of Primary Care First (PCF). Karu Medical Associates and many physicians nationwide are working together to reach the goal of the Triple Aim—improving patient care by increasing quality, improving medical outcomes in a cost effective manner.
Care Managers educate patients on their chronic medical conditions. The Care Manager, the Primary Care Provider (PCP), the patient and/or caregiver, work as a team to develop a personalized plan of care that works for the patient. In Care Management, the patient learns steps they can take to help manage their conditions and engage in shared decision-making.
Care Managers assist patients to navigate the complex healthcare system and can match patients with appropriate services in the community based on their needs.
Care Managers follow-up with patients who have been in the emergency room or hospitalized making sure they have all the services, information, and medications that they need. Also, they ensure timely follow up with the PCP to avoid post-hospital complications.
Care Management is available to all our patients. Should you need help managing your diabetes, hypertension, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), depression, or if you struggle with your weight, Care Management may be right for you.
